Works with individuals experiencing symptoms of mental
illness who require support to live and work in the community * case
managers provide on-going support as needed in order to stabilize,
achieve goals, and improve quality of life * case Management services are
tailored to the individual, based on their identified goals for
support * participation in the Case Management Program is a prerequisite
to obtain CMHA Huron Perth supportive housing Services offered include, but are not limited to:
