| Eligibility - Population(s) Served: | Adults who live in the community, at Baycrest, other long-term care homes and assisted living residences in Ontario. Appointments may be in-person or virtual at the discretion of the care team in collaboration with the patient. | ||
| Application: | Referral from a physician or nurse practitioner is required for many programs and should be initiated through our Central Navigation. | ||
