Description of Services:	Provides services that promote physical and mental health for community members Coordinates health care services through a health team which consists of Community Health Representative, Community Wellness Worker and a Family Support Worker, visiting Physicians and Crisis Team workers Provides health awareness and counselling, education and prevention workshops/clinics in areas that may include Child Care, Diabetes, Elderly & Chronic Disease, Immunization, Sexual Health and Communicable Diseases Coordinates basic foot care clinics Identifies and attempts to correct health hazards Addresses individual health care needs by providing: Intake services to determine health care needs of individual Treatment of on-going health problems Referrals to appropriate health care providers based on information in the intake assessment Advocacy on behalf of individual with other health professionals Coordination of one-on-one pre-natal care with the community health nurse Home visits to the sick, elderly, new mothers and persons with health care needs Assistance with coordinating local and long distance travel arrangements for medical appointments Services and Programs: Aboriginal Diabetes Initiative Aboriginal Healing and Wellness Strategy Brighter Futures Building Healthy Communities Canada Prenatal Nutrition Program Community Food Educator Community Health Representative Community Wellness Worker Program Early Childhood Development Family Support Program Family Violence Initiative Family Well Being Program HIV / AIDS Initiative Medical Transportation Program Non-Insured Health Benefits Program Events Coordinator
