Description of Services:
  • Offers a multi-disciplinary health team of Physicians, Nurse Practitioners and Registered Practical Nurses dedicated to providing quality primary care and walk-in services
  • Provides access to other health care professionals within and outside of the Clinic
  • Coordinates documentation if patient receives care from a member of the team
  • Provides house calls to individuals unable to attend the clinic or hospital
  • Coordinates visiting specialist appointments in the areas of orthopedics, eyes, ears and throat, and oncology
  • Participates in teaching with a regular rotation of medical students and residents from Northern Ontario School of Medicine (NOSM)
    • Offers primary health care including but not limited to:
    • Chronic disease management
    • Health promotion
    • Disease prevention
    • Nutritional counselling
    Requirements
    Fees: Service - Some tests, forms and/or services (such as pilot medicals) are not covered by Ontario Health Insurance Plan (OHIP)
    Eligibility - Population(s) Served: Individuals and families in Sioux Lookout and surrounding area
