Description of Services: Provides services to individuals with an intellectual disability such as:

  • develop and follow up on a person centered plan
  • help individuals adapt to a new environment
  • develop support teams and linkages with other community resources
  • provide services for interim case management for external/internal referrals
  • help individuals transition from a home to an independent environment
  • help individuals manage their funds, accessing other relevant services, etc
