Description of Services: Assists people living on low incomes, suffering from chronic health conditions, facing mental health issues, isolation and/or language barriers

Services include:
  • development of individualized care plans addressing health, housing, safety needs and referrals to appropriate service providers
  • assistance completing applications for Canada Pension Plan, Ontario Disability Support Program, Ontario Works, passports, other identification
  • supportive counseling
  • advocating with case workers, housing and government agencies for effective solutions

Eligibility - Population(s) Served: Adults 55 years and older and adults 18 and older with disabilities, who need physical, social, or emotional supports
