Description of Services: Hospital-based outpatient rehabilitation for people who have complex acquired brain injuries, musculoskeletal injuries, or have had a stroke * services provided by neuropsychologists, nurses, occupational therapists, physiatrists, physicians, physiotherapists, psychologists, rehabilitation therapists, social workers, speech language pathologists, and massage therapists

Fees: Set fee * services may be covered by extended health benefits, auto insurance, and WSIB
Eligibility - Population(s) Served: People who have had a stroke, or have sustained multiple fractures, multiple system injuries, or moderate to severe acquired brain injuries
Application: Medical referral required
