Description of Services: Offer different support groups and outreach initiatives:

Chinese Diabetes Support Group - support provided in Mandarin and Cantonese by both peers and DECNET diabetes educators * facilitated by diabetes educators * third Monday of each month 

Diabetes in the Real World (DRW) - advocacy Group - group of community members who meet regularly to raise awareness about issues faced by people living with type 2 diabetes. * Program currently on hold 

English Diabetes Support Group - an informal environment in which group members can receive support from both peers and DECNET diabetes educators * facilitated by diabetes educators 

Mental Health & Diabetes Group - peer support and diabetes education group that focuses on mental health challenges and managing diabetes * facilitated by diabetes educators 

Outreach Programs/ Initiatives - outreach initiatives that work towards connecting with individuals who may experience barriers to health and health care access and are impacted by diabetes

Eligibility - Population(s) Served: Adults 18 years and older with pre-diabetes and type 2 diabetes
