Supports include:
  • access to rent supplement to obtain suitable housing
  • individual and group teaching of skills related to activities of daily living
  • connections to community services and resources
  • consultation and clinical support services
  • access to mental health nurses
  • development of crisis plans to identify personal coping strategies
Requirements
Eligibility - Population(s) Served: People with serious mental illness who are homeless or have a history of homelessness * must be able to live independently and wish to work toward recovery
