Services include:
  • timely access from ED or Primary Care Provider to the first clinic visit
  • education about AFIB and its management
  • access to necessary cardiac investigations
  • comprehensive management of AFIB and other comorbidities
  • timely access to cardiology, electorphsiology, or specialty care if needed
  • seamless transition to community care providers with 24-48 hours of visit
