Description of Services: Offers assistance to patients who are transitioning from hospital to home

  • Assistance with transportation
  • Home configuration
  • Shopping
  • Finding other community support services to help

Eligibility - Population(s) Served: Individuals 55 years and older who:

  • are able to direct own care or caregiver

  • are medically stable

  • are able to be left unattended

  • are mobile, ambulatory (able to self-transfer), and not need the assistance of more than one person to manage stairs

  • pose no threat to the health and safety of self or workers AND

  • do not live in a long-term care home or Assisted Living where 24-hour care is provided
Application: Potential clients are identified by the hospital or via Ontario Health atHome - North West Area and bookings must be received a minimum of 24 hours in advance
