| Description of Services: | Offers subsidized independent units with on-site support services to individuals transitioning out of high-support housing, in partnership with CAMH. . An on-site community space is available for tenants to engage in community events. The program is recovery-based, and assists tenants in living independently with support. Tenants have access to residential support, assistance with medication management, supportive counselling, peer support and socio-recreational activities. | ||
