Description of Services: Provides an initial interdisciplinary virtual assessment is conducted and the team sets goals with the patient and tailors the program to meet their unique needs *Supports to individuals with COVID-19 who continue to experience difficulties in the following areas:
  • endurance
  • balance
  • thinking skills
  • shortness of breath
  • speech and swallowing
  • managing difficult emotions
  • managing fatigue
Requirements
Fees: Covered by OHIP
Eligibility - Population(s) Served: Medically stable adults with a confirmed COVID-19 diagnosis who are experiencing lingering symptoms of the virus
Application: Only accepting referrals from University Health Network physicians and/or University Health Network programs

Accessibility Notes: Wheelchair accessible building including main entrance and barrier free washrooms * tone elevator, lowered buttons * designated and street parking close to entrance
