This service is available to men and women living with a serious mental illness who are 16 years of age or older and are homeless, or at significant risk of becoming homeless. | ||

| Application: | Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment. | ||
