  • Parents/Guardians of school-aged children requiring consent to receive vaccines at school-based immunization/vaccine clinics in the Thunder Bay District Health Unit area

  • Under the Immunization of School Pupils Act, individuals under the age of 16 must have either signed or verbal parental consent; if within 48 hours of school immunization clinic, consent must be given verbally by calling 807-625-5908
