Description of Services: Supports for people living with gestational, prediabetes or type 2 diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse, registered dietitian, social worker, endocrinologist and diabetes educator on staff
  • non-insulin and insulin injection
  • nutritional counseling
  • foot screening/assessment
  • therapeutic counseling for clients and families
  • ongoing management of chronic diseases
  • triage service for urgent client illness

Eligibility - Population(s) Served: Adults 18 years and older with prediabetes type 2 diabetes
Application: Call * no referral required * un-rostered patients accepted * wait times 1-3 days
