  • Provides individuals with an intellectual and/or developmental disability the opportunity to live and share in the activities of a family home
  • Utilizes a range of group home and apartment settings in the community where families or individuals share their home and time
  • Provides support workers to assist with recreational activities, medical appointments and to ensure the individual is receiving adequate support in the home

    Eligibility - Population(s) Served: Individuals 18 years of age and older with an intellectual and/or developmental disability
