Description of Services: Provides services for community seniors to enable them to remain healthy and live independently as long as possible
  • Offers home visits by a Registered Nurse or Nurse Practitioner following an initial home assessment
  • Monitors medical concerns, illness prevention and offers health promotion
  • Provides nutrition assessments to enable advice about healthy eating and signs of aging such as osteoporosis
  • Assists in obtaining the Lifeline program, an emergency help line reachable at the push of a button
  • Provides referrals to therapists and geriatric experts
