  • Works to increase early detection and to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and self-management of diabetes
  • Promotes increased physical activity and knowledge of healthy eating
  • Provides workshops, presentations and education for school children and community members
  • Provides services of a chiropodist
  • Offers individual, group and school counselling sessions

  • First Nation individuals living with any type of diabetes

  • Individuals with impaired glucose tolerance and/or impaired fasting glucose

  • High risk individuals
