Description of Services:	Education and services for the prevention, management and treatment of diabetes Works to increase early detection and to reduce the number of avoidable complications that can arise from uncontrolled diabetes Advises on developing and implementing an approach to care and self-management of diabetes Promotes increased physical activity and knowledge of healthy eating Provides workshops, presentations and education for school children and community members Provides services of a chiropodist Offers individual, group and school counselling sessions

Eligibility - Population(s) Served:	First Nation individuals living with any type of diabetes Individuals with impaired glucose tolerance and/or impaired fasting glucose High risk individuals

Application:	Appointments recommended No referral required
