Eligibility - Population(s) Served:	Male and female survivors of sexual assault, aged 16 years of age and older The sexual assault must have happened in Ontario

Application:	Complete the voucher request form, available on the website Send the completed voucher by email to ILAvoucher@ontario.ca, OR Print and mail the completed voucher to: Independent Legal Advice Ministry of the Attorney General OVS, Centralized Programs Delivery Unit 720 Bay St, 5th Floor Toronto, ON M7A 2S9
