Description of Services: Support for people living with pre-diabetes or diabetes
  • Offers education and self-management support to fit an individual's lifestyle
  • Provides access to a Registered Nurse and Registered Dietitian
Provides outreach services to First Nation clients living with diabetes in the following areas:
  • Education
  • Monitoring and screening
  • Diabetes compliance, prevention and intervention
  • Networking and liaison with the Dryden Regional Health Centre
  • Referral and liaison to other programs as required

Eligibility - Population(s) Served: First Nation individuals living in the communities within the Dryden Tribal Area with pre-diabetes, type 1 and type 2 diabetes
